新生兒腹瀉是由多種病原多種因素引起的,以大便次數增多和大便性狀改變?yōu)樘攸c的一組消化道綜合征【1】。主要是由于新生兒消化系統(tǒng)發(fā)育不成熟,生長發(fā)育快,機體防御功能差,腸道菌群失調等因素造成的【2】。因此,在積極治療疾病的同時,給予積極完善的護理干預措施可以促進患兒的康復。對我兒科2013年1月-2013年12月收治的新生兒腹瀉患兒給予護理干預措施,取得滿意效果,現將護理干預措施報道如下。

1 資料與方法

1.1 一般資料

選取2013年1月-2013年12月在我兒科治療的新生兒腹瀉患兒240例作為研究對象,隨機分成對照組和觀察組,對照組120例,其中男78例,女42例;年齡6d-24d,平均年齡14.1±0.6d;腹瀉次數5-14次,每次45-190ml。觀察組120例,其中男74例,女46例;年齡11d-23d,平均年齡15.3±0.6d;腹瀉次數4-15次,每次40-180ml。所有患兒入院后均經大便常規(guī)檢查顯示,白細胞(++-+++),脂肪球(+-++),均符合新生兒腹瀉的診斷標準【3】。所有患兒家屬均簽署知情同意書。兩組患兒在年齡、性別、腹瀉次數方面無統(tǒng)計學意義(P>0.05),具有可比性。

1.2護理方法

1.2.1 對照組給予常規(guī)護理措施。

1.2.2.1 嚴密觀察患兒病情變化

患兒入院后嚴密觀察患兒的體溫、呼吸、脈搏等生命體征,準確記錄患兒大便的次數、顏色、形狀等,有無低鉀、低鈣等電解質紊亂癥,有無脫水及其脫水的程度,對于脫水程度不重的患兒給予口服補液鹽,服用方法:體重(kg)*75ml=用量ml/d;對于脫水嚴重且伴有代謝性酸中毒的患兒給予靜脈補液,補液時準確計算補液總量補液成分和補液速度。

1.2.2.2 飲食護理

要根據患兒的腹瀉、嘔吐、食欲和消化情況,確定飲食的方法,對嘔吐、腹瀉嚴重患兒可暫時禁食6-8h,以利腸道休息,減少嘔吐及腹瀉次數,一般不禁水,行母乳喂養(yǎng)的新生兒應持續(xù)喂哺母乳;對于人工喂養(yǎng)的腹瀉較重的新生兒,要減少奶量并用水稀釋,可用1份牛乳加1/2份水,待腹瀉好轉后逐漸增加至全奶。當患兒腹瀉好轉時,可以按照從稀到稠、從軟到硬的規(guī)律逐漸過渡的原則,少量多餐。

1.2.2.3 健康教育

首先宣傳母乳喂養(yǎng)的優(yōu)點,避免在夏季斷乳;新生兒的奶瓶用前應煮沸消毒20分鐘,吃剩的奶液應倒掉;母親應保持乳房的清潔,勤換內衣,盡量減少感染的機會;對于母乳不足可采取混合喂養(yǎng)及人工喂養(yǎng)的辦法,但是要注意飲食調配,避免發(fā)生碳水化合物消化不良,影響寶寶生長發(fā)育;同時注意新生兒腹部的保暖,避免受涼。

對比分析兩組患兒的治療效果和患兒家屬滿意度。

無創(chuàng)機械通氣是急性左心衰患者的有效輔助治療方法之一,可明顯改善患者的呼吸狀況,保證有效通氣,可明顯緩解患者的急性左心衰狀況,改善心肌功能,進而明顯提高患者的生活質量,改善患者的預后[1-2]。為了探討護理干預在急性左心衰患者使用無創(chuàng)機械通氣治療中的影響,本院選取2011年11月~2013年11月診治的急性左心衰患者142例,均行無創(chuàng)機械通氣治療,根據護理方案分為兩組,行常規(guī)護理和護理干預,針對兩組患者的主訴恐懼情況、主訴配合情況、主訴并發(fā)癥情況進行對比分析,現報告如下。

1.資料與方法

1.1資料

本院2011年11月~2013年11月診治的急性左心衰患者142例,年齡為47~81歲,平均年齡為67.3±5.9歲,其中男性108例,女性34例。患者均符合急性左心衰的診斷標準,且經臨床表現和實驗室檢查確診,排除患有其他器質性病變、血液病、免疫性疾病的患者,均行無創(chuàng)機械通氣治療。根據護理方案分為兩組,常規(guī)護理患者71例為對照組,護理干預患者71例為觀察組,兩組患者的基礎資料(性別、年齡等)差異均無統(tǒng)計學意義(p>0.05),具有可比性。

1.2方法

觀察組患者實施護理干預:⑴基礎干預。護理人員采用和藹可親的語言和患者進行交流,建立良好的醫(yī)患關系,取得患者的信任,通過溝通與交流,幫助患者消除焦慮、抑郁、恐懼、害怕、緊張、擔心等不良心理因素,幫助患者建立配合治療的信心和決心,建立良好的干預基礎。⑵情緒干預。向患者講解疾病的相關知識和治療的有效方法,使患者了解到疾病治療的目的和意義,使其可以正視疾病的治療,在治療過程中,患者出現恐懼感時要給予及時的安慰和鼓勵,采用支持性語言和肢體語言穩(wěn)定患者的情緒,提高患者的耐受性,消除患者的不安狀況,減輕患者的恐懼病癥。⑶行為干預。指導患者在治療過程中采用端坐位,將頭部與肩頸處于同一水平位置,將頭后仰,確保呼吸道通暢,佩戴合適的面部口鼻罩,調節(jié)松緊度,設定吸氣相壓力由6cmH2O開始,設定呼氣相壓力由2cmH2O開始,增到患者的耐受,要確保患者的面部皮膚無受損,并在皮膚接觸部位放置紗布,減輕患者的壓迫感,在皮膚接觸部位涂抹凡士林,3h更換1次,每次20min,給予患者局部皮膚按摩等。⑷病情監(jiān)護。注意觀察患者的呼吸節(jié)律和心率狀況,確保主動呼吸和呼吸機同步,注意保持患者的呼吸道通暢,在加濕器內添加蒸餾水,將氣體濕化后進入氣道,指導患者掌握正確的呼吸方法。對照組患者實施常規(guī)護理?;颊咴谥委熯^程中,給予患者常規(guī)的基礎護理,保證患者的病房通風良好和環(huán)境的整潔衛(wèi)生,及時更換患者的床單被褥,只要患者實施正確的飲食和作息等。

臨床上,輪狀病毒感染性腹瀉在兒科中具有較高的發(fā)病率,引發(fā)本病的病原體主要為輪狀病毒,此病的主要臨床癥狀表現除了腹瀉之外同時普遍并發(fā)有脫水、發(fā)熱等癥狀,嚴重的甚至出現代謝紊亂以及酸中毒,通常好發(fā)于秋冬季節(jié),若無法及時診斷并接受針對性治療,嚴重的甚至可直接引發(fā)死亡[1]?,F階段,臨床上對本病的發(fā)病機制尚無法明確,因而無統(tǒng)一的且具有明顯效果的治療標準[2]。為此,我院引入西咪替丁與干擾素聯合微量元素鋅治療,現將療效報道如下。

1 資料與方法

1.1 一般資料

67例均為我院自2015年11月~2016年12月期間收治的輪狀病毒感染性腹瀉患兒,納入標準:臨床癥狀均符合本病的癥狀表現,同時經病原學檢查確診。經醫(yī)院倫理委員會審批后,將67例患兒隨機分為治療組與對照組兩組,對照組32例,其中男17例,女15例;年齡0.7~4.5歲,平均年齡(1.6±0.8)歲。治療組35例,其中男19例,女16例;年齡0.8~4.4歲,平均年齡(1.6±0.7)歲。兩組患兒在基線資料方面對比差異無統(tǒng)計學意義(P>0.05),具有可比性。

1.2 方法

兩組均給予常規(guī)檢查,同時結合脫水的嚴重程度給予補液治療,給予發(fā)熱以及嘔吐者對癥治療,注意兩組均不能給予抗生素治療?;诖?,對照組采用西咪替丁注射液(湖南漢森制藥股份有限公司;H43021799)10~15mg/(kg?d)治療,給藥方式為靜脈滴注,同時增加重組人干擾素α1b [安徽安科生物工程(集團)股份有限公司;S20040010]治療,給藥方式為注射,劑量為100萬U/次,1次/d。在此基礎上,治療組聯合葡萄糖酸鋅口服液(湖北福人金身藥業(yè)有限公司;H20065022)治療,給藥方式為口服,每支的含鋅量為0.03g,1支/d,兩組的療程均為10d。

1.3 觀察指標及評定標準

記錄兩組大便及體溫恢復時間、脫水糾正時間。記錄兩組面色潮紅、煩躁以及腹瀉加重等不良反應情況。對兩組治療效果進行評估,若3d后,癥狀消失,同時糞便次數以及形狀均正常則評定為顯效;若3d后癥狀明顯緩解,同時糞便次數及形狀明顯好轉則評定為有效 ;若3d后癥狀未見任何緩解,同時糞便次數及形狀未見任何改善則評定為無效??傆行?100%-無效率。

1.4 統(tǒng)計學分析

使用SPSS19.0統(tǒng)計學軟件處理數據,兩組計數資料對比行χ2檢驗,兩組計量資料對比行t檢驗,用P

2.1 兩組治療基本情況比較

治療組治療總有效率為97.14%,明顯高于對照組的71.88%(P

2.3 兩組不良反應比較

治療組不良反應發(fā)生率為2.86%,與對照組的不良反應發(fā)生率6.26%對比差異無統(tǒng)計學意義(P>0.05),詳見表3。

3 討論

臨床上,感染性腹瀉屬于兒科中常見的一種腸道感染性疾病,出現腹部疼痛以及糞便不成形等為本病的主要癥狀表現[3-4]。人體小腸內部的載膜絨毛上皮細胞中存在一種可以與輪狀病毒外殼中含有的VP4相互結合的病毒原體,因此小腸成為了輪狀病毒侵入的重要部位[5]。待病毒繼續(xù)侵入機體上皮細胞之后,便會誘發(fā)機體內的大量非結構蛋白糖基化,進而引發(fā)細胞膜發(fā)生紊亂現象,例如鈣離子通透性,一旦機體細胞內的鈣離子出現大量增多的現象,便會引發(fā)高鈣低氯癥狀,進一步可增加病毒非結構蛋白NSP3抑制蛋白正常合成的風險,進而將影響小腸上皮細胞的運行,嚴重的甚至可導致其脫落,極大的減少了雙糖轉化為單糖過程中所需的腸載膜上絨毛酶的數量,隨著時間的延長,雙糖大量堆積在腸道內,便極易增加腸腔內滲透壓,進而誘發(fā)腹瀉、嘔吐及脫水等癥狀,嚴重的可并發(fā)水電解質紊亂以及酸中毒等嚴重威脅患兒生命的癥狀[6-7]。

西咪替丁是一種H2受體拮抗劑,能夠顯著抑制因組胺或者胃泌素引發(fā)的胃酸分泌,進而避免受損腸黏膜長期處在酸性環(huán)境中而對其起到保護作用,并有利于為絨毛的修復創(chuàng)造良好條件[8-9]。此外,此種藥物還具有免疫調節(jié)作用,能夠通過提高患兒機體內的T淋巴細胞的轉化率而起到增強其免疫力的效果,進而能夠避免炎癥反應誘發(fā)的一系列繼發(fā)性損傷問題[10-11]。干擾素則具有免疫調節(jié)以及抗病毒效應等多種生理功能,在本病的治療過程中具有顯著的價值[12]。刷狀緣中大部分酶類均對鋅元素存在較強的依賴性,同時鋅離子能夠顯著的抑制小腸黏膜的增生,且能夠促進其完全角化,進而能夠對腸黏膜起到保護作用,此外,還可抑制胃腸激素分泌,增加腸內鈉氯離子的吸收能力,進而能夠對腸腔的滲透壓起到降低的效果[13]。除此之外,鋅離子還具有免疫調節(jié)以及抑菌等多種功效。特別值得關注的是,我院在采用西咪替丁與干擾素常規(guī)治療的基礎上增加微量元素鋅的一個理論基礎是此類型的患兒血清中鋅代謝有異常的現象[14-16]。

綜上,將西咪替丁與干擾素聯合微量元素鋅應用于輪狀病毒感染性腹瀉患兒的治療中,可以快速消除患兒的癥狀,促進其康復,具有療效理想,安全性高的特點,值得臨床上推廣及應用。
